CVS Health Securing the Future of Pharmacy in Louisiana Scholarship

CRITERIA FOR SELECTION:

To qualify for the scholarship, a student must meet the following:

A. Recipient(s) must be a full-time student who has been admitted or is enrolled in the ULM College of Pharmacy’s Doctor of Pharmacy (PharmD) program.

B. Be a resident of the State of Louisiana; or graduate from a Louisiana High School; or attended a Louisiana institution of higher learning.

C. Be a U.S. citizen or lawful permanent resident.

D. Be in good academic standing.

E. Recipient(s) must meet university standards for acceptance in the University of Louisiana Monroe.

F. Recipient(s) must be able to demonstrate a financial and/or personal need.

G. This scholarships intent is to assist students who have a financial need as evidenced by the Student Aid Report (SAR) received by the university after the FAFSA process is complete each year and who might otherwise be unable to attend college without this scholarship.

H. Participate in one College of Pharmacy recruitment activity during the year of award.

I. At end of awarded year, student will draft a statement answering, “How the award of these funds impacted their academic and professional career”.

J. After graduation, student will participate as an Alumni Ambassador for a period of one year.

IV. SELECTION OF RECIPIENT:
The College of Pharmacy will send recommended recipient(s) name(s) to the University of Louisiana Monroe Scholarship committee for final selection(s).
